I also supply the link in the rental contract. I set the multiple page stuff to 4 per page and print out the whole form on one page, front/back. It is still readable with my laser printer.

http://www2.epa.gov/sites/production/files/documents/lesr_eng.pdf I just have them sign the form above Julie.. I know they just throw the pamphlet away.
